1. I certify that I am at least 18 years of age and hold a valid and current driver's license, and that I have chosen to participate in or observe certain activities taking place at the New River Valley Truck Plant in Dublin, Virginia, which may include but not necessarily be limited to riding in and/or driving a truck or trucks manufactured by Volvo Group North America, LLC ("VOLVO"). I understand that I may be riding in and/or driving vehicles over extreme terrain and/or at high speed in order to learn about the capabilities of the VOLVO products, and that as a passenger or driver I will be subjected to high speeds, severe shocks, vibrations and cab movements (the "ACTIVITY"). I agree to follow all instructions, rules and posted speed limits associated with the ACTIVITY.

2. I acknowledge that there are inherent dangers related to the ACTIVITY which may result in loss or damage to my person or property, or result in my death. I declare that I am of sound health and know of no impediment that would prevent me from safely undertaking the ACTIVITY. I agree that I will not participate in the ACTIVITY if I am under the influence of drugs or alcohol, if am pregnant, or if there is any other mental or physical condition that may impair my ability to understand instructions or to participate without creating risk to others or myself.

3. I acknowledge that I have been informed that my personal safety cannot be guaranteed. I hereby voluntarily assume any and all risks of injury, death, loss or damage to my person or property while I am using the property, facilities and services of VOLVO, or observing or participating in any way in the ACTIVITY. In addition, I declare that I am financially responsible for any death or injury that may occur to me during or as a result of such observing or participation.

4. IN RETURN FOR MY BEING PERMITTED TO PARTICIPATE IN ANY PART OF THE ACTIVITY, I HEREBY RELEASE, WAIVE AND DISCHARGE VOLVO, AND ITS EMPLOYEES, AFFILIATES, REPRESENTATIVES, SUCCESSORS AND ASSIGNS FROM ANY AND ALL CLAIMS, DEMANDS OR LIABILITY WHICH MAY ARISE FROM MY PARTICIPATION IN THE ACTIVITY.

I have entered into this AGREEMENT on the basis of my own information and not in reliance upon representations of VOLVO and/or any other released parties. I further understand that this document contains the entire agreement and no oral or written agreements limiting or modifying the effect of the terms of this AGREEMENT exist.

I INTEND THAT THIS AGREEMENT IS AND WILL BE BINDING ON MY FAMILY, ESTATE, HEIRS, SUCCESSORS, ASSIGNS, INSURERS, MEDICAL PROVIDERS AND PERSONAL REPRESENTATIVES.

By providing my information below, I represent that I have knowingly and voluntarily submitted this AGREEMENT with the intent that it be a legally binding document.
